Choose the conditions you want to file for and complete your disability claim application.

Important Note About Medical Records
Many veterans were discouraged from going to medical while in service. Because of this, your service treatment records may not show documentation for every condition you’re claiming.
Do not let that stop you from filing a claim.
If a condition is not clearly documented, submit a personal statement describing:

What happened during service

When the condition started
-
How it has continued since service
Your statement helps connect your condition to your military service.
Common Conditions to Consider Claiming
(You can absolutely add anything else you’re experiencing.)
Mental Health

Describe stressors experienced during service

If claiming PTSD, you must identify a specific, verifiable stressor
Tinnitus

State that it started during military service (if applicable)

Explain that it has continued ever since
Hearing Loss

Describe loud noise exposure during service

Include frequency and duration (hours per day, type of noise, etc.)

State that you have not had loud noise exposure since leaving service
Headaches / Migraines

How often they occur

How long they last
-
How you treat them (lying down, medication, dark room, etc.)
Back Issues

It’s often best to be general and allow the VA to do a full exam

If you have radiculopathy (pain, numbness, tingling down arms or legs), be sure to mention it
